2 A RESOLUTION OF THE CITY C01.TNCIL OF THE CITY OF VERNON REQUESTING
THE WORKMEN'S COMPENSATION APPEALS BOARD OF THE STATE OF CALIFOR-

5 THE CITY COUNCIL OF THE CITY OF VERNON RESOLVES AS FOLLOWS:
6 WHEREAs, the City Council of the City of Vernon is
7 advised and believes that RICHARD C. ZINDEL, a Local Safety Mem-
8 ber, should be retired for disability, and
9 WHEREAS, neither the CITY OF VERNOR, as Employer, nor
10 the Member have agreed upon the e:ffecti.ve date of the disability
11 retirement or whether said disability is permanent and stationary
12 NOW, THEREFORE, BE IT RESOLVED:
13 SECTION 1: That the City Council of the City of Vernon
14 does hereby request from the Workmen's Compensation Appeals
15 Board of the State of California a determination as to whether
16 RICHARD C. ZINDEL, a Local Safety Member, has received a work-
17 incurred disability, and whether the same is permanent and sta-
18 tionary and, if so, the date when said disability became perma-
19 nent and stationary.

SECtION 2: That the City Council of the City of Vernon
21 authorizes the :filing of an application on such form or forms as
22 may be necessary with the Workmen's Compensation Appeals Board
23 to request the determination as to whether RICHARD C. ZINDEL'S
24 disability is industrial.

SECTION 3: The City Clerk of the City of Vernon shall
26 certify to the passage of this Resolution and thereupon and
27 thereafter the same shall be in full force and effect.
28 ADOPTED AND APPROVED this 18th day of December, 1973.
