1 RESOLUTION NO. 5820
2 A RESOLUTION OF THE CITY COUNCIL OF THE
CITY OF VERNON APPROVING AND AUTHORIZING
3 THE EXECUTION OF AN AMENDMENT NO. 2 TO
ADMINISTRATIVE SERVICES AGREEMENT (GUSI-
4 2R04 AND GUSI-2R05) BY AND BETWEEN THE
CITY OF VERNON AND UNITED OF OMAHA LIFE
5 INSURANCE COMPANY (OMAHA)
6 WHEREAS, the City Council of the City of Vernon adopted
7 Resolution No. 5813 on August 21, 1990, declaring its intention
8 to renew the Master stop Loss Policy and Administrative Services
9 Contract with Omaha; and
10 WHEREAS, the city Council of the City of Vernon adopted
11 Resolution No.
5657 on August 22,
1989, approving and
12 authorizing the execution of an Administrative Services
13 Agreement with Omaha providing for the administration of
14 Vernon's health care benefit plan;
15
WHEREAS, Omaha wishes to amend said Agreement to
16 provide for an increase in its administration fee to be
17 effective September 1, 1990.
18 NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF
19 THE CITY OF VERNON AS FOLLOWS:

SECTION 1: The City Council of the City of Vernon
21 hereby finds and determines that the recitals contained
22 hereinabove are true and correct.

SECTION 2: The City Council of the city of Vernon
24 hereby approves the Amendment No. 2 to Administrative Services
25 Agreement, effective September 1, 1990, a copy of which has been
26 presented to the City Council concurrently with this resolution
27 and the city Council hereby orders said Amendment No. 2 to be
28 received and filed by the City Clerk.

SECTION 3: The City Council of the City of Vernon
hereby authorizes the Mayor and the City Clerk to execute said

Amendment No. 2 to Administrative Services Agreement, for, and
on behalf of, the City of Vernon.

certify to the passage of this resolution and thereupon and
SECTION 4: The City Clerk of the City of Vernon shall
thereafter the same shall be in full force and effect.

AMENDMENT NO.2
TO
ADMINISTRATIVE SERVICES AGREEMENT
(GUSI - 2R04 AND GUSI - 2R05)
Effective September I, 1990, this Amendment No.2 to the Administrative Services Agreement
of September 1, 1989 between City of Vernon (Employer) and United of Omaha Life Insurance
Company (United) is made a part of such Agreement and is subject to the provisions of the
Agreement not in conflict with the provisions of this Amendment.
Employer and United mutually agree that Provision 5.1.1 is deleted from the Agreement and the
following substituted:
5.1.1 10.42% of the face amounts of all Plan Benefit Account checks issued each month by
the Company.
