A RESOLUTION OF THE CITY COUNCIL OF THE
CITY OF VERNON APPROVING AND AUTHORIZING
THE EXECUTION OF AN ADMINISTRATIVE
SERVICES AGREEMENT (EPSI-4H15) BY AND
BETWEEN THE CITY OF VERNON AND UNITED OF
OMAHA LIFE INSURANCE COMPANY (OMAHA)

WHEREAS, the City Council of the City of Vernon adopted
7
Resolution No. 5820 on september 4, 1990, approving and
8
authorizing the execution of an amendment to the Administrative
9
Services Agreement with Omaha providing for the administration
of Vernon's health care benefit plan; and
WHEREAS, the City of Vernon wishes to enter into
another Agreement with Omaha to provide for administration of
the City's self-insured health plan, to be effective February 1,
1992.
NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF
THE CITY OF VERNON AS FOLLOWS:
SECTION 1: The City Council of the City of Vernon
hereby finds and determines that the recitals contained
hereinabove are true and correct.
SECTION 2: The City Council of the City of Vernon
hereby approves the Administrative Services Agreement, effective
February 1, 1992, a copy of which has been presented to the City
Council concurrently with this resolution, and the City Council
hereby orders said Agreement to be received and filed by the
City Clerk.
SECTION 3:
The City council of the City of Vernon
hereby authorizes the Mayor and the City Clerk to execute said

certify to the passage of this resolution, and thereupon and
