A RESOLUTION OF THE CITY COUNCIL OF THE
CITY OF VERNON APPROVING AND AUTHORIZING
THE EXECUTION OF A PAYOR AGREEMENT (EPSI-
4H15) BY AND BETWEEN THE CITY OF VERNON
AND UNITED OF OMAHA LIFE INSURANCE COMPANY
(OMAHA)

WHEREAS, the City of Vernon and Omaha desire to enter
7
into a written agreement, the Payor Agreement, effective
8
February 1, 1992, to provide the terms and conditions under
9
which Omaha is able to enter into mutually preferred provider
agreements with certain preferred provider organizations andlor
health care providers.
NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF
THE CITY OF VERNON AS FOLLOWS:
SECTION 1:
The City Council of the City of Vernon
hereby finds and determines that the recital contained
hereinabove is true and correct.
SECTION 2:
The City Council of the City of Vernon
hereby approves the Payor Agreement, effective February 1, 1992,
a copy of which has been presented to the City Council
concurrently with this resolution, and the City Council hereby
orders said Agreement to be received and filed by the City
Clerk.
SECTION 3:
The City Council of the City of Vernon
hereby authorizes the Mayor and the City Clerk to execute said
Agreement for, and on behalf of, the City of Vernon.

SECTION 4: The City Clerk of the City of Vernon shall

certify to the passage of this resolution, and thereupon and
3
thereafter the same shall be in full force and effect.

APPROVED AND ADOPTED this 18th day of February, 1992.
