RESOLUTION NO. 6057

A RESOLUTION OF THE CITY COUNCIL OF THE
CITY OF VERNON APPROVING AND AUTHORIZING
THE EXECUTION OF AN AMENDMENT NO. 3 TO
ADMINISTRATIVE SERVICES AGREEMENT (GUSI-
2R04 AND GUSI-2R05) BY AND BETWEEN THE
CITY OF VERNON AND UNITED OF OMAHA LIFE
INSURANCE COMPANY (OMAHA)

WHEREAS, the City Council of the City of Vernon adopted
7
Resolution No. 5657 on August 22,
1989, approving and
8 authorizing the execution of an Administrative Services
9 Agreement with Omaha providing for the administration of
10 Vernon's health care benefit plan;
11
WHEREAS, Omaha wishes to amend said Agreement to
12 provide for an increase in its administration fee to be
13 effective september 1, 1991.
14 NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF
15 THE CITY OF VERNON AS FOLLOWS:

SECTION 1:
The City Council of the City of Vernon
17 hereby finds and determines that the recitals contained
18 hereinabove are true and correct.

SECTION 2:
The City Council of the City of Vernon
20 hereby approves the Amendment No. 3 to Administrative Services
21 Agreement, effective september 1, 1991, a copy of which has been
22 presented to the City Council concurrently with this resolution
23 and the City Council hereby orders said Amendment No. 3 to be
24 received and filed by the City Clerk.

SECTION 3:
The City council of the City of Vernon
26 hereby authorizes the Mayor and the city Clerk to execute said
27 Amendment No. 3 to Administrative Services Agreement, for, and
28 on behalf of, the City of Vernon.

SECTION 4: The City Clerk of the City of Vernon shall
certify to the passage of this resolution and thereupon and

thereafter the same shall be in full force and effect.
