2 A RESOLUTION OF THE CITY COUNCIL OF THE CITY OF
VERNON APPROVING AND AUTHORIZING THE EXECUTION OF
3 AN AMENDMENT TO WORKERS' COMPENSATION SELF-
INSURANCE SERVICE AGREEMENT BY AND BETWEEN THE
4 CITY OF VERNON AND COLEN & LEE, INC. PROVIDING FOR
AN INCREASE IN FEES FOR THE ADMINISTRATION OF A
5 WORKERS' COMPENSATION SELF-INSURANCE PROGRAM
6 WHEREAS, the City of Vernon is self-insured in its workers'
7 compensation program; and
8 WHEREAS, by Resolution No. 5791 the City Council approved a
9 Workers' Compensation Self-Insurance Service Agreement with Colen &
10 Lee, Inc., as administrator for the City's workers' compensation
11 self-insurance program; and

WHEREAS, Colen & Lee, Inc., has requested an amendment to
13 said Service Agreement reflecting an increase from $2,325.00 to
14 $2,700.00 per month for fiscal year 1992-1993; and
15 WHEREAS, the City Administrator has recommended that said
16 increase be approved as being fair and reasonable, and that Colen &
17 Lee, Inc. continue to administer the program for the City of Vernon.
18 NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF THE
19 CITY OF VERNON AS FOLLOWS:

SECTION 1: The City Council of the City of Vernon hereby
21 finds and determines that the recitals contained hereinabove are true
22 and correct.

SECTION 2: The City Council of the City of Vernon hereby
24 approves the Amendment to Workers' Compensation Self-Insurance
25 Service Agreement, a copy of which has been presented to the City
26 Council concurrently with this resolution, and the City Council
27 hereby orders said Amendment to be received and filed by the City
28 Clerk.
1 SECTION 3: The City Council of the City of Vernon hereby
2 authorizes the Mayor and the City Clerk to execute said Amendment
3 for, and on behalf of, the City of Vernon.
4 SECTION 4: The City Clerk of the City of Vernon shall
5 certify to the passage of this resolution, and thereupon and
6 thereafter the same shall be in full force and effect.
7 APPROVED AND ADOPTED this 18th day of August, 1992.
