A RESOLUTION OF THE CITY COUNCIL OF THE CITY OF
VERNON APPROVING AND AUTHORIZING THE EXECUTION OF
AN AMENDMENT TO LIABILITY SELF-INSURANCE
AGREEMENT WITH COLEN & LEE, INC.

WHEREAS, the City Council of the City of Vernon by
Resolution No. 5320 on September 16, 1986, authorized a Liability

Inc.
for the
Self-Insurance Agreement with Colen & Lee,
7
administration of a general and auto liability self-insurance
8
program; and
9
WHEREAS, said Agreement provided for administrative fees of

$500.00 per month, which fees have been increased by various
purchase orders and are currently $650.00 per month for 1991/1992;
and
WHEREAS, Colen & Lee, Inc. has submi tted a proposed
Amendment to said Agreement establishing the same fee for 1992/1993,
which the City Administrator has recommended as being fair and

reasonable.
NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF THE

CITY OF VERNON AS FOLLOWS:

SECTION 1: The City Council of the City of Vernon hereby
20

finds and determines that the recitals contained hereinabove are
true and correct.
22
SECTION 2: The City Council of the City of Vernon hereby
23
approves the Amendment to Liability Self-Insurance Agreement, a copy
24
of which has been presented to the City Council concurrently with
25
26
this ReSOlution, and the City Council hereby orders said Amendment
to be received and filed by the City Clerk.
SECTION 3: The City Council of the City of Vernon hereby
27

authorizes the Mayor and the City Clerk to execute said Amendment
1 for, and on behalf of, the city of Vernon.

SECTION 4: The city Clerk of the city of Vernon shall
3 certify to the passage of this Resolution, and thereupon and
4 thereafter the same shall be in full force and effect.
5 APPROVED AND ADOPTED this 20th day of October, 1992.

LIABILITY SELF-INSURANCE
SERVICE AGREEMENT
THIS AGREEMENT is entered into this.)!' ('''day of Oe A"N/' -: 1992
between the CITY OF VERNON, hereinafter referred to as the "City." and
COLEN AND LEE INC., a California corporation. hereinafter referred to as
the "Administrator."
1. CONSIDERATION: Section 9 of the Original Agreement is hereby
amended to read: The City shall continue to pay the Administrator
$650.00 per month for ongoing services rendered under this Agreement.
2. ORIGINAL AGREEMENT: Except as specifically amended herein, the
terms and conditions of the Original Agreement shall remain in full
force and effect.
