A RESOLUTION OF THE CITY COUNCIL OF THE CITY OF
VERNON APPROVING AND AUTHORIZING THE RENEWAL
AND EXECUTION OF THE MASTER STOP LOSS POLICY
(GUG-2R03) BY AND BETWEEN THE CITY OF VERNON
AND UNITED OF OMAHA LIFE INSURANCE COMPANY

WHEREAS, the Master stop Loss Policy with united of Omaha
7 Life Insurance Company effective September 1, 1992, provided
8 insurance coverage under the City's self-insured health plan which
9 pays for hospital, surgical and medical expenses above certain
10 limits, was approved by the City Council of the City of Vernon on
11 May 4,1993, by Resolution No. 6251 and terminated on August 31,
12 1993; and
13 WHEREAS, the City of Vernon desires to renew the Master
14 stop Loss POlicy (GUG-2R03) for the period of September 1, 1993
15 through August 31, 1994.
16 NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF THE
17 CITY OF VERNON AS FOLLOWS:

SECTION 1: The City Council of the City of Vernon hereby
19 finds and determines that the recitals contained hereinabove are
20 true and correct.

SECTION 2: The City Council of the City of Vernon hereby
22 approves the Master stop Loss Policy (GUG-2R03), effective September
23 1, 1993, a copy of which has been presented to the city Council
24 concurrently with this resolution, and the city Council hereby
25 orders said Policy to be received and filed by the City Clerk.

SECTION 3: The City Council of the City of Vernon hereby
27 authorizes the Mayor and the City Clerk to execute and accept said
28 Policy for, and on behalf of, the City of Vernon.

SECTION 4: The City Clerk of the City of Vernon shall
2 certify to the passage of this resolution, and thereupon and
3 thereafter the same shall be in full force and effect.
4 APPROVED AND ADOPTED this 21st day of December, 1993.
