RESQLUTIQN NO. 6613
A RESOLUTION OF THE CITY COUNCIL OF THE CITY
OF VERNON APPROVING AND AUTHORIZING THE
RENEWAL AND EXECUTION OF THE MASTER STOP LOSS
POLICY (GUG-2R03) BY AND BETWEEN THE CITY OF
VERNON AND UNITED OF OMAHA LIFE INSURANCE
COMPANY
WHEREAS, the Master stop Loss Policy with United of
Life Insurance Company effective september 1, 1993 to August
8
31, 1994, which provided insurance coverage in excess of specified
9
limits for the City's self~insured health plan for hospital,
surgical and medical expenses, was approved by the City Council of
10

the City of Vernon by Resolution No. 6371 on December 21, 1993;
and
WHEREAS, the City Council by minute order dated
August 30, 1994, approved a continuation of such insurance policy
contingent upon the City Attorney's approval of the final
agreement; and
WHEREAS, the Risk Manager has been able to negotiate a

reduction in rates, effective December 1, 1994; and
19
WHEREAS, the City Attorney has approved the revised
20

agreement; and
WHEREAS, the City Council of the City of Vernon desires
22
to renew the Master stop Loss Policy (GUG-2R03) for the period of
23
September 1, 1994 through August 31, 1995, with the revised rates
taking effect December 1, 1994.

NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF
THE CITY OF VERNON AS FOLLOWS:

SECTION 1: The City Council of the City of Vernon

hereby finds and determines that the recitals contained

hereinabove are true and correct.

SECTION 2: The City Council of the city of Vernon
4 hereby approves the Master stop Loss Policy (GUG-2R03), effective
5 September 1, 1994, with revised rates effective December 1, 1994,
6 a copy of which has been presented to the City Council
7 concurrently with this resolution, and the City council hereby
8 orders said Policy to be received and filed by the City Clerk.

SECTION 3: The City Council of the City of Vernon
10 hereby authorizes the Mayor and the City Clerk to execute said
11 Policy for, and on behalf of, the City of Vernon.
12
SECTION 4: The City Clerk of the City of Vernon shall
13 certify to the passage of this resolution, and thereupon and
14 thereafter the same shall be in full force and effect.
15 APPROVED AND ADOPTED this 18th day of April, 1995.

PROVIDER OPTION ADMINISTRATIVE SERVICES AGREEMENT
(EPSI-4H15) AND AMENDMENT NO.5 TO ADMINISTRATIVE
SERVICES AGREEMENT (GUSI-2R04 AND GUSI-2R05); AND
(2) APPROViNG AND RATIFYING THE EXECUTION OF
AMENDMENT NOS. 1 AND 4 TO ADMINISTRATIVE SERVICES
AGREEMENT (GUSI-2R04 AND GUSI-2R05) BY AND
BETWEEN THE CITY OF VERNON AND UNITED OF OMAHA

WHEREAS, the City Council of the City of Vernon has three
agreements with united of Omaha Life Insurance company (Omaha) for
administering the City's employee health care plan:

Administrative Service Agreement (GUSI-2R04 and

GUSI-2R05) approved by City council Resolution No. 5687 on August

22, 1989, and amended by Resolution No. 5820 on september 4, 1990,

and by Resolution No. 6057 on February 18, 1993.

Exclusive Provider Option Administrative Services
Agree~ent (EPSI-4H15) approved by city council Resolution No. 6053
on February 18, 1992.
3. Exclusive Provider Option Payor Agreement (EPSI-4H15)

approved by city council Resolution No. 6054 on February 18, 1992;

and
WHEREAS, Omaha has submitted Amendment No. 5 to Admini-
strative ~~rvices Agreement (GUSI-2R04 and GUSI-2R05) to provide for
an increase in its administrati~e fees effective september 1, 1993;

and
WHEREAS, Amendment No. 1 to Administrative Services
Agreement (GUSI-2R04 AND GUSI-2R05)
authorized payment of a
consultation fee to Peter H. Cobo Insurance Agency, Inc., effective

September 1, 1989, and was approved administratively on February 5,

1990, by Edward A. Cooney, Risk Manager, but has not heretofore been

presented to the City Council for approval; and
WHEREAS, Amendment No. 4 to Administrative Services
Agreement (GUSI-2R04 AND GUSI-2R05) reinstated payment of fees to

Omaha (after a trans-fer to Mutual Plan Administrators, Inc. was
rejected by the City of Vernon) and was approved administratively on

but has not
