A RESOLUTION OF THE CITY COUNCIL OF THE CITY
OF VERNON APPROVING AND AUTHORIZING THE
EXECUTION OF AMENDMENT NO. 2 TO EXCLUSIVE
PROVIDER OPTION ADMINISTRATIVE SERVICES
AGREEMENT (EPSI-4H15) BY AND BETWEEN THE CITY
OF VERNON AND UNITED OF OMAHA LIFE INSURANCE
COMPANY

WHEREAS, the City Council of the City of Vernon has
8
three agreements with United of Omaha Life Insurance Company
9
("Omaha") for administering the City'S employee health care plan;
and
10

WHEREAS, Administrative Service Agreement (GUSI-2R04 and
12
GUSI-2R05) and Exclusive Provider option Payor Agreement (EPSI-
13
4H15), which were last amended by City Council Resolution No. 6368
14
on January 18, 1994, will continue in full force and effect; and
15
16
WHEREAS, Omaha has submitted Amendment No. 2 to the
Exclusive Provider Option Administrative Services Agreement (EPSI-
17
4H15), last amended by Resolution No. 6368 on January 18, 1994,
18
providing for a decrease in the administrative fees effective
19
December 1, 1994; and

WHEREAS, the City Administrator has recommended approval
of said amendment.

NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF
THE CITY OF VERNON AS FOLLOWS:

SECTION 1: The City Council of the City of Vernon
hereby finds and determines that the recitals contained
25

hereinabove are true and correct.

SECTION 2: The City Council of the City of Vernon
28
hereby approves Amendment No. 2 to Exclusive Provider Option
1 Administrative Services Agreement (EPSI-4H15) effective
2 December 1, 1994, a copy of which Amendment has been presented to
3 the City Council concurrently with this resolution, and the City
4 Council hereby orders said Amendment to be received and filed by
5 the City Clerk.

SECTION 3: The City Council of the City of Vernon
7 hereby authorizes the Mayor and the City Clerk to execute said
8 Amendment for, and on behalf of, the City of Vernon.

SECTION 4: The City Clerk of the City of Vernon shall

certify to the passage of this resolution, and thereupon and
thereafter the same shall be in full force and effect.
APPROVED AND ADOPTED this 18th day of April, 1995.
