A RESOLUTION OF THE CITY COUNCIL OF THE CITY
OF VERNON APPROVING AND AUTHORIZING THE
EXECUTION OF AMENDMENT NO. 8 TO THE
ADMINISTRATIVE SERVICES AGREEMENT (GUSI-2R04,
GUSI-2R05 AND GUSI-9V57) BY AND BETWEEN THE
CITY OF VERNON AND UNITED OF OMAHA LIFE
INSURANCE COMPANY

WHEREAS, the City Council of the City of Vernon has
8
agreements with United of Omaha Life Insurance Company ("Omaha")
9
for administering the City's employee health care plan; and
10
WHEREAS, Omaha has submitted Amendment No. 8 to the
11
Administrative Services Agreement (GUSI-2R04, GUSI-2R05 AND GUSI-
9V57), last amended by Resolution No. 6614 on April 18, 1995,

providing for administration of the City's vision plan and
14
revisions to the administrative fee schedule, effective September
15
1, 1996; and
16
WHEREAS, the City Administrator has recommended approval
17
of said amendment.

NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF
19
THE CITY OF VERNON AS FOLLOWS:

SECTION 1: The City Council of the City of Vernon
21 hereby finds and determines that the recitals contained
22 hereinabove are true and correct.
23
SECTION 2: The City Council of the City of Vernon
24 hereby approves Amendment No. 8 to Administrative Services (GUSI-
25 2R04, GUSI-2R05 AND GUSI-9V57), effective September 1, 1996, a
26 copy of which Amendment has been presented to the City Council
27 concurrently with this resolution, and the city Council hereby
28 orders said Amendment to be received and filed by the City Clerk.

SECTION 3: The City Council of the City of Vernon
2 hereby authorizes the Mayor and the City Clerk to execute said
3 Agreement for, and on behalf of, the City of Vernon.

SECTION 4: The City Clerk of the City of Vernon shall
5 certify to the passage of this resolution, and thereupon and
6 thereafter the same shall be in full force and effect.
7 APPROVED AND ADOPTED this 21st day of October, 1997.
