Resolution No. 7684I RESOLUTION NO. 7684 2 A RESOLUTION OF THE CITY COUNCIL OF THE CITY OF 3 VERNON APPROVING AND AUTHORIZING NOTICE INVITING SEALED PROPOSALS OR BIDS FOR THE PURCHASE OF A NEW 4 TELECOMMUNICATIONS SYSTEM TO REPLACE THE EXISTING ONE 5 6 WHEREAS, the Utilities Department is requesting that the City 7 purchase a new state-of-the-art Telecommunications System to replace 8 the existing outdated Telecommunications System to meet the City's 9 expanding technological needs and provide the City with the cost 10 savings produced by the elimination of various business lines; and 11 WHEREAS, on December 12, 2000, the Finance Committee approved 12 the recommendation of Bruce V. Malkenhorst, the Director of Finance, 13 dated December 7, 2000, that authority be granted for the Purchasing 14 Agent, or his designee, to seek formal bids for a new Tele- 15 communications System to replace the existing system within an 16 approximate cost range of Two Hundred Fifty Thousand Dollars and No 17 Cents ($250,000.00) to Five Hundred Thousand Dollars and No Cents 18 ($500,000.00), depending upon the configuration; and 19 WHEREAS, a Request for Proposal has been prepared for the 20 purchase, installation and maintenance of a new Telecommunications 21 System; and 22 WHEREAS, the City Council of the City of Vernon has 23 determined that it is in the best interests of the City of Vernon that 24 bids or proposals be solicited in accordance with applicable State law 25 and the Vernon City Code. 26 27 28 / / / 1 NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF THE 2 CITY OF VERNON AS FOLLOWS: 3 SECTION 1: The City Council of the City of Vernon hereby 4 finds and determines that the recitals contained hereinabove are true 5 and correct. 6 SECTION 2: The Request for Proposal, a copy of which is 7 attached hereto and made a part hereof as Exhibit "A," is hereby $ approved by the City Council of the City of Vernon. 9 SECTION 3: The City Clerk of the City of Vernon is hereby 10 directed to give notice, in writing, inviting sealed proposals or bids 11 for the purchase, installation and maintenance of a new Tele 12 communications System in accordance with the Request for Proposal, and 13 in accordance with this resolution. 14 SECTION 4: There being no newspaper of general circulation 15 printed, published or circulated in the City of Vernon, the City Clerk 16 is hereby directed to post said notice, or cause the same to be 17 posted, in three of the most public places in the City of Vernon, to 1$ wit: the northwest corner of 38th Street and Santa Fe Avenue, the 19 northeast corner of Leonis Boulevard and Pacific Boulevard, and on the 20 bulletin board in the lobby of the City Hall of the City of Vernon, 21 located at 4305 Santa Fe Avenue, in said City. 22 SECTION 5: The City Clerk of the City of Vernon is hereby 23 directed to post said notice forthwith and the first posting of said 24 notice shall be at least ten (10) days before the opening of the bids 25 for the proposed contract. Purpose of Request for Proposal The City of Vernon (City). is planning to acquire a new state-of-the-art telecommunications system (System) to meet existing telecommunications requirements. The new System must accommodate future enhancement needs, changing technological needs, and organizational growth in the next 10 years. The City has an existing Private Branch Exchange (PBX) system that services City Hall, four fire department facilities, one control center, and two satellite offices outside of the City limits. The City has prepared this Request for Proposal to solicit qualified vendors to provide solutions for furnishing, installing, and maintaining a new System that will satisfy the requirements set forth in this document. In addition, after successful implementation and evaluation, City will consider marketing the System to existing businesses in Vernon. II. Requirements for Bidders 1. This Request for Proposal (RFP) consists of several items that are described herein. Please check to ensure that all items are present. Call Purchasing Department at (323) 583-8811 if any are missing. 2. Complete all applicable sections of the RFP for the systems and options you are proposing. Mark the Bidder Response Form "acknowledge", "exception", or "no bid" to show compliance or none -compliance with this RFP. An alternative and explanation are required after each exception explaining your non-compliance or alteration to this RFP. 3. Only existing systems, along with modifications necessary for compliance, will be considered. Systems under development, in planning, or at test sites will not be considered. 4. The proposed system shall be immediately demonstrable at vendor locations. Vendors will be required to arrange for a system demonstration during the evaluation phase. 5. Proposals (three copies) are due no later than 2:00 p.m. on to: Purchasing Department City of Vernon 4305 S. Santa Fe Avenue Vernon, CA 90058 All proposals shall be labeled with the bid number on the outside envelope for consideration. 6. All proposals should include all information solicited by this RFP plus any additional data, prints and. literature that the.vendor deems pertinent to the . understanding and evaluation of the bid. 7. Any interpretations of or corrections or changes to this RFP will be made by addenda, which will be provided to all vendors. 8. The proposed System must incorporate recent advances in voice and data switching technology and be capable of operating 24 hours a day, seven days a week with no system operator or attendant intervention. The proposed system must be fully digital with specialized software to ensure improved call processing capabilities, and provide a flexible platform to meet changing needs through the next 10 years. The system design should include private Branch Exchange (PBX), voice mail/automated attendant and, automatic call distribution (ACD)..Computer Telephone Interface (CTI) and Interactive Web Services (IWS) functionality will be implemented in the future. 9. Bidders must conform to the response format detailed in this RFP and made on forms provided with all items filled out and properly executed. Copies of all applicable proposed contracts shall be included in the 10. p pp p P response. 11. The complete proposal and response to the RFP must include the following: a. The unit prices of the required and optional equipment items. The bidder is required to use the pricing sheets provided in this RFP. b. The cost of: (1) equipment, (2) labor, (3) materials, and (4) other costs incidental to the cost of installation as indicated on the required pricing sheets. c. Totally completed pricing sheets, signed (where requested) by an authorized representative of the bidder. d. A point -by -point response to the RFP and any Addenda (including appendices) in which each paragraph of the RFP is identified and the bidder's response given. e. A written list of all exceptions (if any) to the requirements of all sections. . f. The legal name of the bidder and a statement as to whether the bidder is a sole proprietor, a partnership, a corporation, or any other legal entity. Evaluation Criteria l� All proposals will be evaluated on the following criteria: 1. The initial cost and projected costs of expansion and maintenance of the proposed system cost and any costs associated with telephone room preparation. 2. Bidder's technical expertise and prior experience in the field including installation, training, and maintenance of systems and projects of comparable size, diversity, and complexity. 3. The size, structure, and stability of bidder's organization. 4. The bidder's adherence to bid instructions and requirements and the bidder's understanding of the work to be performed. 5. The bidder's guarantee in writing that all essential parts for proper maintenance of the equipment being offered are available at the execution of the contract and will continue to be available for the following ten (10) years. 6. The bidder's guarantee of the availability of the bidder's personnel who are factory -trained and certified on the proposed system to provide technical service on an as -needed basis. 7. The evaluation of references provided by the bidder. 8. The technical design and performance record of the proposed system. 9. The manufacturer's stability and commitment to the proposed product(s). 10. The ability of all existing or planned system capabilities to meet the City's planned or future needs. 11. The reliability and serviceability of the proposed system based upon the manufacturer's published specifications and user references. 12. The degree of modularity, expandability and flexibility and redundant options in the architecture of the proposed system. These three qualities will enable the system to be quickly re -configured in response to a changing business environment and to be economically expanded to its maximum limits. 3 13. The City may require bidders to give a presentation of their product offerings. The presentation shall be no longer than two hours. The bidders shall include the following information: a. System demonstration. b. System design and capabilities. C. Required system features. d. Statement of the proposed System's reliability and maintainability. e. Optional systems and features. f. Pricing. 14. The expandable ability to adapt to a fiber optic service from a provider. 15. The expandable ability to support video conferencing applications. 16. Demonstrated (preferred) or calculated statements of mean -time -before - failure (MTBF) and mean -time -to -repair (MTTR) for each of the proposed equipment. Scope of Work This RFP is for an integrated voice/data telecommunication system (System) to serve the City of Vernon (City) facilities. It is the desire of the City that the System be able to support all present and planned applications and ancillary systems as specified in this RFP. As integral elements of this project, the selected vendor shall: 1. Design, install, test, cutover, and warranty for a minimum of one year from the date of cutover of the System (PBX and automated attendant), specified elsewhere herein. 2. The system shall interface without any interruption of service with the existing 911 system as specified elsewhere herein. 3. Conduct all required station reviews, and develop and install in the System all required system operating and user databases. 4. Develop and conduct station user and operator training on site and provide user manuals specifically designed for the System being installed. 4 V. 5. Provide comprehensive "system administration" technical training on the System to certify three (3) City employees to perform all typically required moves, adds, and changes software programming. 6. The selected vendor shall provide an experienced Project Manager to be the City's single point of contact and he shall have complete responsibility for the installation, cutover, training, and documentation. The Project Manager shall be on site at all times when vendor's technicians or subcontractors are on site. Bidders shall provide the resume of their Project Manager with their bid responses. 7. Vendors shall propose a fully functional System; a "turnkey" solution, including coordinating and placing orders with a telecommunications service provider, cabling and required plant work. 8. Traffic Study Bidder shall perform a traffic study for seven consecutive days after cutover. Bidder shall balance lines during the study. Reports of the study and rearrangements must be provided in writing after study is conducted. System Requirements 1. Location and Service a. The System will be located in City Hall at 4305 Santa Fe Avenue, Vernon, CA. The bidders are required to inspect the telephone room where the System will be installed. b. Each bidder must submit a detailed scale drawing of the System showing the installation and placement of the System in the ' telephone room. The scale drawing will identify work space, and power, heating and cooling requirements to operate'the System reliably. City will provide the required power, heating and cooling services. C. The successful bidder will coordinate and place any orders necessary to install all operating telecommunications facilities from a service provider to operate the System including long distance and common carrier. The telecommunication service will support the PBX configuration to provide the services identified in Exhibits A and B of this RFP. Vendors are required to verify the information provided in the exhibits. d. The telecommunications service shall consist of trunks and lines listed below: 5 1. Direct- Inward -Dialing (DID). 2. Direct -Outward -Dialing (DOD). 3. T-1 carrier channel. 4. ISDN PRI. 5. T-3 carrier channel. Bidders shall propose trunking and lines necessary to support the existing telecommunication needs shown in Exhibits A and B. The System proposed must be able to adapt to a fiber optic service from a provider. Bidders shall provide options for pricing the System and the installation cost for a fiber optic service to the City. e. The bidders will coordinate and place an order for the City for trunk lines to provide reliable back up service in case the primary trunk service fails or be rendered out of service. The back up trunk line will be used to support the City's emergency telecommunications needs for police, fire, control center and other critical City Hall functions. The bidders shall identify and provide all the necessary devices that will be installed at all the sites identified in Exhibits A and B of this RFP. All telecommunication services will be routed through City Hall. Remote locations (four fire departments and the control center) will be connected to the System using the City -owned fiber optic cables. The bidders will be responsible for. providing the switches and connecting the remote sites to the PBX. g. The bidders shall assess the re -useability of the City's existing cable plant. The bidders shall identify new cabling needs or any modifications necessary to provide a successful System installation. Bidders shall incorporate sections 2 through 23 and shall be marked as Bidder Response Form. h. Proposed System must be a fully functional offering, i.e. a "turnkey" solution, including required cable and plant work. Vendor is to provide detailed specification recommendations for inside and outside plant and cable. Specify for each station cable run of the proposed design how wires are allocated for voice, data, and spares. 6 2. System Overview Please present a general overview and configuration of the proposed System. This overview should include how the PBX, ACD or peripheral systems are configured and how each system will integrate with the other(s). Please include the following information: Manufacturer: Model # Designation: Software Release: Describe the increments by which the proposed System's line/trunk and station capacities can be expanded. Indicate the breakpoints at which additional cabinets, common equipment, and/or software license fee will be required with the appropriate costs. Describe the methods/requirements to upgrade the system to connect to fiber optic system from the service provider. Identify components, hardware and software to be replaced or upgraded with appropriate costs. 3. . Digital Station Features Please describe your complement of station features available as standard equipment. Please describe features available on optional or executive station equipment. 4. Switch Architecture and Features Describe the proposed System switching architecture Describe the maximum port size capacity of the proposed system. The vendor must guarantee that expansion will be accomplished by adding printed circuit cards. All cabinets, carriers, power supplies, etc. must be included in the initial installation. Acknowledge Exception No Bid List and describe the major components and software required to reach the maximum port size. The System must be modular to enable expansion to full capacity without replacement of any significant amount of previously installed hardware. Acknowledge Exception 7 Vendor shall describe the method and steps of expanding the proposed System beyond the proposed initial capacity. Acknowledge Exception No Bid Please describe the programmable and non -programmable System features available as standard equipment. 5. Reliability and Redundanc For this application, redundancy is defined as the capability of an additional system component (of equal functionality and capacity a failed component) to automatically and immediately take over for a failed system component upon such occurrence. The bidders must discuss and provide optional pricing for the following: All levels of system redundancy available on the proposed System. The vendor must supply a detailed explanation of system reliability and/or redundancy in the following critical areas of the System. a. Common control circuitry, including the central processor and switching matrix; b. Component shelves, card slots, and power and communication busses; C. Power supply components; d. Service circuits such as call progress tone sources and ringing generators. List redundant components in the proposed configuration. Specify the demonstrated (preferred) or calculated mean -time -before - failure (MTBF) and mean -time -to -repair (MTTR) for each of the proposed equipment elements and state the impact of such a failure on the operation of the System. The redundancy feature shall include automatic switchover to the backup system if a fault is detected. Describe the effect a switchover has on voice and data users when they are placing a call, receiving a call, or while connected. Does the System automatically switch to the backup systems each day to insure they are in working order? Explain the process. Describe how the System is monitored remotely and the notification process. Describe how alarm conditions are reported. Describe any variable alarm thresholds. 6. Traffic Capacity Please describe the proposed System's traffic handling capacity. The System shall be essentially non -blocking and shall include sufficient time slots, software and hardware capacity to provide minimum busy hour traffic handling capability for the proposed configuration. Acknowledge Exception No Bid 7. Diagnostics The System shall include necessary hardware and software to perform diagnostic tests to evaluate the System. The System shall run these tests periodically without manual startup and upon command from a local or remote maintenance terminal Acknowledge Exception - No Bid The System shall provide for local and remote terminal access to diagnostic capabilities. Diagnostic tests shall run concurrently with all system call processing activities without affecting the System's traffic handling capabilities. Acknowledge Exception No Bid The System must have over -temperature detection capability, notification capability, and a power down mechanism. Acknowledge Exception .-No Bid The System shall log automatically all detected hardware and software faults. System Software and Administration The - System Administrator will have on site, direct remote, or data switched access for administration functions. Describe the levels available. Describe changes that require vendor assistance to accomplish. Include copies and descriptions of trunk, feature and console usage reports, as well as diagnostics reports that are available on the proposed System. 9. Data Capabilities The proposed System's data communications capability shall include an open architecture. Please describe. Describe how the system administration can communicate with active data communications users to coordinate computer maintenance. 11 On outgoing calls, the System must automatically select an available port with the correct characteristics. It will then use the lease cost routing feature to place the call over the most economical circuit. Acknowledge Exception No Bid 10. Power and Batteries To protect against voltage fluctuations and power outages, the vendor shall provide an Uninterruptible Power Supply (UPS) sufficient to operate the System for 2 hours upon loss of main AC power. Acknowledge Exception No Bid The UPS shall be designed to be fault tolerant (i.e. dual chargers and inverters). The chargers shall be of the ferro resonant type. Acknowledge Exception No Bid The UPS shall be continuously on-line, and, during normal operation, maintain power to the System, and maintain the batteries at full rated charge. The UPS shall be equipped with make before break transfer switch for System maintenance. Acknowledge Exception No Bid The UPS shall be equipped with a transfer switch to allow for independent testing of batteries during system maintenance. Acknowledge Exception No Bid Describe the type of fuse protection offered with the System. Provide a schematic drawing to show all major components of the internal and external power -distribution system. Indicate which components are duplicated. 11. ACD The proposed switch must include a digital Automatic Call Distributor. Call Accounting System Provide complete description of Call Accounting System for station billing including all system, user features, benefits, and application and training. Acknowledge Exception No Bid 14. Distinctive Ringing The System must be able to provide distinctive ringing on the telephones to distinguish external calls and from internal calls. Acknowledge Exception No tjia 15. Night Service Incoming calls to the main listed number will normally be routed to the operator during the day. In the night mode, calls will be routed to an alternate answering point to a recorded announcement device. Acknowledge Exception No Bid 16. Restrictions The System Administrator shall be able to program to restrict any station from Direct -Inward -Dial, Direct -Outward -Dial, and toll dialing. Acknowledge Exception No Bid 17. Internal Dialing The PBX shall provide a four digit or less internal dialing code. Acknowledge Exception No Bid 18. Dialing Codes a. The PBX shall have the following single digit dial access codes:, 1) "O" — Operator 2) "9" — Outside line access. b. The System Administrator shall be allowed to assign codes to users to allow toll dialing. Acknowledge Exception No Bid 14 19 20 Call Forwarding/Call Transfer All stations shall be capable of transferring all calls to any other extension in the System. Acknowledge Telephones and Consoles a 31 C. Operator Console Exception No Bid A desk -top operator console capable of either handset or headset operation shall be provided. All calls directed to the console shall provide both visual and audible signals. The console shall be able to perform all functions normally required which include: "O" level calls, recalls, calls to listed directory numbers, the capability to complete outward and inward calls, transfer calls, split calls, camp - on override and paging. Console training shall be provided one week before system cutover. Acknowledge Exception No Bid Administrative Station Console/Telephone Set Each department identified in Exhibits A and B will have a department administrative support. All calls to a station will automatically roll-over to the designated administrative telephone/ console or be directed to the voicemail messaging. The administrative console or telephone set will be capable to support up to 30 phone numbers. All calls and roll-over to the administrative stations shall provide visual and audible signals. The console shall be capable of handset, headset, or speaker operation, calls to listed directory numbers, complete outward and inward calls, transfer calls and camp -on. Training shall be provided. Acknowledge Exception No Bid Telephones All telephones will be touch-tone. All stations will be capable of completing outward and inward calls, transfer calls and camp -on. Video Conferencina The City is considering a system to support video conferencing applications between City Hall and other work sites. Describe the system's capability to support this application. The System shall be integrated with DISPATCH Works® as specified below: DISPATCHWorks® can interface digitally to the System using a Voicebridge Technologies VTG card. The Voicebridge VB-1007 card emulates the functions of a Mitel Superset 430, Northern Telecom 2616, or AT&T 7405 set. The transfer function included in DISPATCH Works® is handled by the PBX via telephone set button emulation. The HOLD, PRIVACY, and CONFERENCE functions are DISPATCH Works® function. Numerous softkey functions can be emulated by adding buttons to the desktop. Acknowledge Exception No Bid 23. The System shall support the reverse 911 System functions as specified below: Reverse 911 is an automatic fax broadcast notification system that requires loop start analog telephone lines in a hunt group for rollover capabilities for use with outbound calling. The system set up is as follows: 1 Dedicated Diagnostics/Outbound 6 Dedicated or seized Outbound only 2 Dedicated w/ hunt group Inbound/Outbound . Acknowledge. Exception No Bid Vendor Installation, Service and Support Policies 1. Maintenance What is your present maintenance hourly labor rate for time and materials service? For preventive maintenance? For moves and changes? Does 16 this include travel time from your closest dispatching station? Where is this dispatch station? Maintenance must be supplied 24 hours a day, 7 days a week, and this price must be included in your proposal in the cost of the new System for the first year. The vendor must maintain current and up-to-date documentation (hardware, software and cabling), for the proposed system, on site and in a single secure location to be specified by the City. Such documentation shall include: descriptions and manuals for the proposed system, hardware configurations, drawings of system configuration, commented software program listings with functional descriptions, cable runs and records, and panel connections and wiring layouts, and shall be made available to specified City employees and representatives. Please describe the expedite procedure that the customer may follow in the event of a service problem. Please specify trouble duration and levels of expedite. 2. Manufacturers Support If the vendor is a distributor and not a manufacturer of any of the systems proposed, insert below the necessary letters or documents from the manufacturers stating their obligation to supply parts, engineering Support and service for the specified systems. 3. Disaster Plan In the event any of the telephone system is destroyed or made totally inoperable as a result of a major disaster, the selected vendor will employ all applicable resources to correct such incident with a minimum of delay. Please describe the steps your company would take to provide either a replacement system or interim service. 4. Cutover and Acceptance Upon completion of the installation, vendor shall test the System for conformity with the RFP requirements. At such time as all tests have been completed, vendor shall give the City written notice thereof, together with its certification that all applicable standards and specifications have been met, and written notice of the date of which the System is to be placed into service (e.g. "cutover'). On the date of cutover, the City, provided the System. is operating according to applicable standards and specifications, shall sign an acknowledgment stating that cutover has occurred. It is understood that 17 vendor's warranty will begin at cutover. It is also understood that if satisfactory cutover and System operation have not occurred, the City's refusal to sign for System acceptance will delay the release of funds. 5. Service and Maintenance Include a description of the standard warranty contracts that apply to the System being proposed and specify the length of the warranty period. Describe the warranty on peripheral hardware or software products, if difference than proposed system warranty. Specify any additional maintenance or support services available during the warranty term. Include literature describing such services and their associated prices. 6. Post Warranty Support Services Describe all available maintenance plans, including preventive maintenance. Supply maintenance agreement specifying response targets as part of this RFP indicating any discounts available to the client if purchased in advance. 7. Definitions Service coverage hours will coincide with the City's hours of operation. Critical services such as Fire, Police and Control Center are 24 hours, 7 days a week operation. Emergencies are defined as: a. System failure. b. Failure of a common control unit. C. Failure of a power supply.. d. Failure of a trunk circuit. e. Failure of 10% or more of user base. f. Failure of a console. g. Failure of communications affecting Fire, Police, and Control Center functions. 18 I Vendor shall respond to emergencies and during the coverage period within 2 hours. Vendor shall respond to minor outages during the coverage period within 24 hours. 8. Moves. Adds, and Changes (MAC) How long after cutover are installation prices in effect? Provide separate pricing for routine software MAC. If pricing is based on an hourly rate, indicate the rate used, and provide pricing assuming that the quantity of MAC orders shall equal 10% times the quantity of equipped ports. 9. Service Alternatives Please describe what, if any, service responsibilities the City may assume for a reduction in the service pricing, for example performing preventive maintenance. Here the vendor may wish to suggest cost effective ways of servicing the System. Also indicate additional available services and associated charges. 10. Service Commitment Briefly outline the capabilities of the servicing organization. Vendor may wish to comment on the number of locally available repair and operations personnel, on the vendor's parts distribution system (nearby parts depot location, availablility of spares, etc.), and on any other areas that demonstrate the vendor's service commitment. If subcontractors are to be used, please provide their name, qualifications, and functions to be performed. 11. Training Submit a training plan and instruction manuals for users, attendants. and administrators. Indicate what training is included in the price of the System, what training support is available after the initial cutover, and any charge for such training. Also, submit a plan to train three City personnel to become system administrators to enable them to make moves, adds, and changes, and to replace PBX station and trunk cards after the first and to include instruction manuals. The cost of this training and instruction manuals must be included in the purchase price of the new System. 19 12. Pr oiect Organization The vendor shall submit a proposed schedule of activities to include a PERT network for the entire project. Include each major component and task with an estimate of the start and completion dates showing the connectivity between tasks and identify the critical path. Vendor is to provide a detailed implementation plan and an installation team organizational chart including job functions and number of people required. VII. Contracts The accepted proposal will be incorporated into the contract that the City will enter into with the vendor. Vendor shall provide insurance in the amounts and forms specified in Exhibit C before contract is signed by the City. VIII. Installation 1. Permits and Licenses Bidder will be responsible for obtaining and paying for all necessary permits and licenses required for the installation of the System. 2. The bidder shall keep the working area free from debris at all times. All rubbish resulting from any work being done by bidder shall be removed from City premises on a daily basis. 3. Bidder shall take all necessary steps to protect City's building areas where bidder is working. Bidder shall be liable and responsible for any repairs required due to negligence by bidder. The general liability policy shall contain the following special endorsements which shall be noted on or attached to the standard certificate of insurance: 1. An endorsement naming the City of Vernon, its officers, and employees as insureds under the policy. 2. An endorsement providing the City of Vernon thirty (30) days notice of cancellation or material reduction of coverage. 3. An endorsement providing coverage for all operations under City of Vernon Contract Number 4. Such other endorsement as may be required by addendum hereto. b. In addition to the standard certificate of insurance, proof of general- and umbrella liability coverage shall be furnished in the form checked below. Certification of the follownq_proofs by the insurance accent or broker will not be accepted: For each policy, a notarized letter from the underwriter or carrier certifying that the coverage and statements in the standard certificate of insurance (attached thereto) are true and correct and that the signator is, an officer authorized to so certify.
